How to Apply for Funding

Thank you for your interest in the Upper Cumberland Affiliate Grants Program.

To apply for a grant:

  1. Make sure you meet the eligibility requirements
  2. Choose the opportunity that best meets your organization's program, Large or Small Grant
  3. Attend the Grant Writing Workshop (this is optional but beneficial)/ read the Community Profile
  4. Submit an application

To be eligible to apply for a Komen Community-Based Grant, your organization must be:

• A federally tax-exempt entity — nonprofit organization, government agency, educational institution or Indian tribe.
• Located in the specified affiliate service area.
• Provide breast health or breast cancer screening, treatment, education or outreach

Obtain and download either the Small Grants Application or the Request for Applications (RFA) for a Large Community Grant.  

• Carefully review the RFA or Small Grants Application to see if you have any questions about its requirements.
• Make sure to submit your completed RFA by the deadline.
• If you are applying for a Small Grant, please submit your completed application at least 6 weeks prior to your event or travel.

Large Grant Funding Opportunities ($5,001 to $25,000)- our Community Grant application process is now CLOSED Please check back in October 2012

The Upper Cumberland Affiliate of Susan G. Komen for the Cure® is currently offering grants from $5,001 to $25,000 for innovative projects in the areas of breast health and breast cancer education, screening, and treatment support targeting services not otherwise available to the medically underserved populations of the Upper Cumberland. Grants are available for up to one (1) year. You must fill out an RFA for a large grant. read more

Small Grant Funding Opportunities- ($1 to $5,000) open year around

The Komen Upper Cumberland Affiliate is currently offering grants up to $5,000 to support breast health education projects, conference, and travel scholarships. Small Grants Applications are accepted throughout the year.  However, requests for support of travel scholarships, conferences, or other events must be received at least six weeks prior to the event. read more 

Upper Cumberland Susan G. Komen holds an annual Grant Writing Workshop at no cost. It is held in November each year. Please e-mail Faune at faune@komenuppercumberland.org if you are interested in attending this workshop. Reservations are required. The 2011 Workshop was held on November 9th.

Collaborative Partnerships: If your organization is a for-profit, there is a way you can participate in a breast health project funded by the Komen Upper Cumberland Affiliate. You may partner with an eligible nonprofit. Collaboration between community organizations is encouraged, but not required, for a successful grant. Collaboration between community organizations often increases a grant's efficiency in reaching targeted populations and access to services.

Examples of potential partnerships might include: A collaboration to share institutional capacity, such as a large community institution (e.g., a hospital, county health department, health care foundation, college or university) and one or more grass-roots community organizations working cooperatively to effectively deliver services to a target population in specific locations in the community. For example, a non-profit senior citizen center could partner with a for-profit hospital to provide free mammograms. The non-profit senior center would be the grantee.

Community Profile

The Affiliate relies on the Community Profile (CP) to guide its work. The CP involves a local overview of demographic and breast cancer statistics that highlight target areas, groups, and issues. These statistics help identify where efforts will have the most impact. In order to ensure effective and targeted efforts, it is important to understand existing programs and services, as well as gaps, needs, and barriers that exist at present in our particular area. The CP also includes an analysis of the local community achieved by including the voices of those individuals living in the Upper Cumberland region and representing priority populations. The CP will use the evidence based conclusions as a guide to make necessary changes throughout the community to improve breast health.
