Important Dates for Large Grantees

Forms for Large Grants of 2012
All large grant recipients are required to fill out a mid-year and final report.

Both forms, progress report and individuals served, must be filled out entirely and submitted to grants@komenuppercumberland.org no later than the due date.
Grantees are given half of the money awarded to start their program. After successful evaluation of the mid year report, the rest of the awarded money will be given to complete the program. Upper Cumberland Affiliate staff and appointed volunteer liaisons are available to assist grantees as needed.
In addition to reporting, site visits may be conducted during program activities. Site visits may be conducted by staff, board members, grant committee members, or grantee liaisons.
If you need to change the role of personnel, budget items, dates, or anything listed in your grant application please fill out a request form. All changes must be approved. Please submit the form to grants@komenuppercumberland.org.
